3.0T磁共振波谱与动态增强扫描联合应用诊断乳腺癌的价值

摘要 摘要目的探讨3.0T磁共振波谱成像(1H-MRS)、动态增强扫描(DCE-MRI)及其联合应用对乳腺癌的诊断价值.方法我院2013年3月至2015年8月磁共振资料完整并经穿刺活检或手术病理证实的乳腺肿瘤患者123例,其中乳腺恶性肿瘤67例,良性肿瘤56例,回顾分析所有氢质子波谱及动态增强扫描数据,与病理结果对照,比较单独使用1H-MRS、DCE-MRI以及二者联合应用诊断乳腺癌的敏感性、特异性及诊断准确率.数据采用X2检验统计分析.结果单独以动态增强扫描的时间-信号强度曲线类型单独诊断乳腺癌的敏感性为83.58%,特异性为62.50%,准确率为73.98%;单独应用1H-MRS,以是否出现胆碱峰(Cho)来诊断乳腺癌的敏感性为52.24%,特异性为87.50%,准确率为68.29%;DCE-MRI联合1H-MRS诊断乳腺癌的敏感性为95.52%,特异性为91.07%,准确率为93.50%.结论单独应用DCE-MRI诊断乳腺癌的敏感性高,但特异性相对较低,良恶性病变表现之间存在重叠;单独应用1H-MRS特异性较高,但敏感性相对较低;二者联合应用可提高诊断敏感性、特异性及准确率.关键词磁共振成像;动态增强;磁共振波谱Thevalueof1H-MRSandDCE-MRIcombinedapplicationinthediagnosisofbreastcancerLiZhen1,ZouYue-fen1,WangXiao-hui2,ZhenXiao-feng2(1.Thefirstaffiliatedhospitalofnanjingmedicaluniversity,nanjing,jiangsuprovince,210029;2.Beijingaerospacegeneralhospitalimagecenter,BeiGjing100076)AbstractObjectiveTostudythevalueof3.0TMagneticResonanceSpectroscopy,(1H-MRS)、DynamicContrast-EnhancedMRI(DCE-MRI),anditscombinedapplicationindiagnosisofbreastcancer.Methods123casesbreasttumors(67malignantand56benigntumor)werecollected,whichwereconfirmedbypathologicalafterbiopsyorsurgical.Allpatientsundergoingroutinescan,DCE-MRIafterinjectionofcontrastagentand1H-MRSexamination,TimesignalIntensityCurve(TIC)typeandpresenceofabnormalincreaseofcholinepeak(Cho)wereanalyzed,comparedwiththepathologicalresults.usingX2-testasstaGtisticalmethods,computationaldiagnosticsensitivity,specificityanddiagnosticaccuracyofbreastcancer.ResultsDiagnosisbreastcancerwithTICtype,thesenGsitivitywas83.58%,specificitywas62.50%,theaccuracywas73.98%.With1H-MRS,thesensitivitywas52.24%,specificitywas87.50%,theaccuracywas68.29%.withDCE-MRIjoint1H-MRS,thesensitivitywas95.52%,specificitywas91.07%,theaccuracywas93.50%.ConclusionApplicationofsignalDCE-MRI,thesensitivityishigherinthediagnosisofbreastcancer,butthespecificityisrelativelylower,theoverlapbetweenbenignandmalignantlesionsperGformance;Separateapplication1H-MRSspecificityishigher,butthesensitivityisrelativelylower;Thejointapplicationcanimprovethediagnosticsensitivity,specificKietyyawnodrdasccuracy.MagneticResonanceImaging(MRI)DynamicContrast-Enhanced(DCE-MRI)MagneticResonanceSpectroscopy(MRS)BreastCancer中图分类号R737.9文献标识码B文章编号1008-6315(2015)10-0302-02
