摘要
摘要目的探讨３．０T磁共振波谱成像(１H－MRS)、动态增强扫描(DCE－MRI)及其联合应用对乳腺癌的诊断价值.方法我院２０１３年３月至２０１５年８月磁共振资料完整并经穿刺活检或手术病理证实的乳腺肿瘤患者１２３例,其中乳腺恶性肿瘤６７例,良性肿瘤５６例,回顾分析所有氢质子波谱及动态增强扫描数据,与病理结果对照,比较单独使用１H－MRS、DCE－MRI以及二者联合应用诊断乳腺癌的敏感性、特异性及诊断准确率.数据采用X２检验统计分析.结果单独以动态增强扫描的时间－信号强度曲线类型单独诊断乳腺癌的敏感性为８３．５８％,特异性为６２．５０％,准确率为７３．９８％;单独应用１H－MRS,以是否出现胆碱峰(Cho)来诊断乳腺癌的敏感性为５２．２４％,特异性为８７．５０％,准确率为６８．２９％;DCE－MRI联合１H－MRS诊断乳腺癌的敏感性为９５．５２％,特异性为９１．０７％,准确率为９３．５０％.结论单独应用DCE－MRI诊断乳腺癌的敏感性高,但特异性相对较低,良恶性病变表现之间存在重叠;单独应用１H－MRS特异性较高,但敏感性相对较低;二者联合应用可提高诊断敏感性、特异性及准确率.关键词磁共振成像;动态增强;磁共振波谱Thevalueof１H－MRSandDCE－MRIcombinedapplicationinthediagnosisofbreastcancerLiZhen１,ZouYue－fen１,WangXiao－hui２,ZhenXiao－feng２(１．Thefirstaffiliatedhospitalofnanjingmedicaluniversity,nanjing,jiangsuprovince,２１００２９;２．Beijingaerospacegeneralhospitalimagecenter,BeiＧjing１０００７６)AbstractObjectiveTostudythevalueof３．０TMagneticResonanceSpectroscopy,(１H－MRS)、DynamicContrast－EnhancedMRI(DCE－MRI),anditscombinedapplicationindiagnosisofbreastcancer．Methods１２３casesbreasttumors(６７malignantand５６benigntumor)werecollected,whichwereconfirmedbypathologicalafterbiopsyorsurgical．Allpatientsundergoingroutinescan,DCE－MRIafterinjectionofcontrastagentand１H－MRSexamination,TimesignalIntensityCurve(TIC)typeandpresenceofabnormalincreaseofcholinepeak(Cho)wereanalyzed,comparedwiththepathologicalresults．usingX２－testasstaＧtisticalmethods,computationaldiagnosticsensitivity,specificityanddiagnosticaccuracyofbreastcancer．ResultsDiagnosisbreastcancerwithTICtype,thesenＧsitivitywas８３．５８％,specificitywas６２．５０％,theaccuracywas７３．９８％．With１H－MRS,thesensitivitywas５２．２４％,specificitywas８７．５０％,theaccuracywas６８．２９％．withDCE－MRIjoint１H－MRS,thesensitivitywas９５．５２％,specificitywas９１．０７％,theaccuracywas９３．５０％．ConclusionApplicationofsignalDCE－MRI,thesensitivityishigherinthediagnosisofbreastcancer,butthespecificityisrelativelylower,theoverlapbetweenbenignandmalignantlesionsperＧformance;Separateapplication１H－MRSspecificityishigher,butthesensitivityisrelativelylower;Thejointapplicationcanimprovethediagnosticsensitivity,specificKietyyawnodrdasccuracy．MagneticResonanceImaging(MRI)DynamicContrast－Enhanced(DCE－MRI)MagneticResonanceSpectroscopy(MRS)BreastCancer中图分类号R７３７．９文献标识码B文章编号１００８－６３１５(２０１５)１０－０３０２－０２
