Thisarticlediscussestheadequatetreatmentofearlygallbladdercancer(T1a,T1b)andisbasedonpublishedstudiesextendingovernearly3decades.Randomizedstudiesandmetaanalysescomparingdifferentsurgicaltreatmentsdonotexist.Theliteratureshowsthatinupto20%ofpatientslymphnodemetastasisarefoundinT1bgallbladdercancer.Duetohighmalignancywithearlyangiolymphaticspreadandresistancetochemotherapyandradiationontheonehand,andtherelativelowoperativeriskofextendedcholecystectomy(cholecystectomyandregionallymphadenectomy)ontheotherhand,webelievethatthisprocedureismandatoryinearlygallbladdercancer.
