简介：双侧甲状腺癌临床比较少见[1],病理类型以乳头状癌及滤泡状癌多见。本文回顾分析2009年1月~2014年1月外科治疗的43例双侧甲状腺癌患者的临床资料,就其手术方式及并发症的预防进行总结。1资料与方法1.1资料患者43例,其中男性9例、女性34例;年龄19~80岁,平均50.3岁;病程1d~20年,平均17.7个月。

简介：目的探讨鼻内途径治疗慢性泪囊炎。方法采用鼻内窥镜下经鼻腔进路,去除部分鼻粘膜、鼻骨,将泪囊内侧壁切开造孔,从而解决鼻泪管阻塞,治疗泪溢、流脓。结果35例慢性泪囊炎经该术式全部取得成功。结论腔镜下经鼻泪囊鼻腔造孔术是治疗慢性泪囊炎的一种有效方法。

简介：目的评价不增加切口的20G标准三通道玻璃体手术治疗严重眼外伤脉络膜上腔出血的疗效。方法对2011年10月至2014年10月间我科收治的29例（29眼）严重眼外伤合并脉络膜上腔出血的病人采取玻璃体手术治疗,置灌注后,通过板层的20G玻切穿刺口引流脉络膜上腔积血,再通过此切口行玻璃体手术,处理玻血和或网膜损伤,术终眼内填充气体或硅油。结果25例患者术中脉络膜上腔出血充分引流完全复位,4例患者残留少量积血,表现为小块脉络膜隆起,注入硅油后,术后一月复查网膜复位。所有患者术后视力均有改善。结论采用不增加切口的三通道玻切的巩膜切口可以有效引流外伤性脉络膜上腔出血,达到理想疗效。

简介：AIM:Toevaluatetheefficacyandsafetyoftrabeculectomy,phacotrabeculectomyplusintraocularlensimplantation(phacotrab+IOLgroup)andphacoemulsificationwithIOL(phaco+IOL)inprimaryangle-closureglaucoma(PACG).METHODS:Itwasasystematicreviewandmeta-analysis,randomizedcontrolledtrials(RCT)andclinicalcontrolledtrials(CCT)werecollectedthroughelectronicsearchesoftheCochraneLibrary,PubMed,EMbase,WanfangDatabaseonline,ChinesejournalFull-textDatabase,ChineseScientificJournalsFull-textDatabase(fromthedateofbuildingthedatabasetoOctober2010)Wealsocheckedthebibliographiesofretrievedarticles.Alltherelateddatathatmatchedourstandardswereabstracted.ThequalityofincludedtrialswasevaluatedaccordingtotheDutchCochraneCentre.RevMan5.0softwarewasusedforMeta-analysis.RESULTS:Atotalof5RCTand11CCTinvolving1495eyeswereincluded.Theresultsofmeta-analysisshowedthatphacotrab+IOLgroupwassuperiorthantrabeculectomy(trabgroup)(MD-3.93,95%CI[-7.31,-0.54])whichwasalsosuperiorthanphaco+IOLgroup(MD0.52,95%CI[0.10,0.95])indecreasingIntraocularPressure(IOP).Phacotrabgroup(MD-1.45,95%CI[-1.68,-1.22])andphacogroup(MD-1.12,95%CI[-1.87,-0.37])arebothdeeperthantrabgroupintheanteriorchamberdepth.Inincreasingthecoefficientofoutflowfacilityofaqueoushumor(Cvalues)therewasnostatisticaldifferenceinthethreegroups.Andtherewasnostatisticaldifferencebetweenphacotrabgroupsandphacogroupsinvisualacuitybutphacotrabgroupwassuperiorthanphacogroup(MD1.07,95%CI[0.73,1.40])intheuseofIOP-loweringdrugs.Therewasnostatisticaldifferenceamongthreegroups.CONCLUSION:Currentevidencesuggeststhatphacotrab+IOLgroupwassuperiorthantrabgroupwhichwasalsosuperiorthanphaco+IOLgroupindecreasingIOP.Phacotrabgroupandphacogrouparebothdeeperthantrabgroupintheanteriorchamberdepth.PhacotrabgroupwassuperiorthanphacogroupintheuseofIOP-loweringdrugs.

简介：AIM:Toreporttheeffectivenessandsafetyofprimary23-Gauge(G)vitreoretinalsurgeryforrhegmatogenousretinaldetachment(RRD).·METHODS:Inthisretrospectivestudy,49eyesof49consecutivepatientswhounderwentprimary23-Gtransconjunctivalsuturelessvitrectomy(TSV)forRRDbetweenJanuary2007andJuly2009atourinstitutionwereevaluated.·RESULTS:Meanfollow-uptimewas8.9±7.7months(1-28months).Retinalreattachmentwasachievedwithasingleoperationin47(95.9%)of49eyes.Intwoeyes(4.1%),retinalredetachmentduetonewbreakswassuccessfullytreatedwithreoperationusingthe23-GTSVsystem.MeanlogMARvisualacuitywas2.01±0.47preoperativelyand1.3±0.5postoperatively(P<0.001,Pairedt-test).Meanpreoperativeintraocularpressure(IOP)was14.1±2.8mmHg.MeanpostoperativeIOPwas12.3±3.6mmHgat1day,13.1±2.1mmHgat1week,14.3±2.2mmHgat1month.Iatrogenicperipheralretinalbreakwasobservedin1eye(2.0%)intraoperatively.Nosutureswererequiredtoclosethescleralorconjunctivalopenings,andnoeyesrequiredconvertionofsurgeryto20-Gvitrectomy.·CONCLUSION:Primary23-GTSVsystemwasobservedtobeeffectiveandsafeinthetreatmentofRRD.

简介：目的探讨鼻内镜结合电子鼻咽喉镜在鼻窦手术中的应用。方法对32例上颌窦侧壁与底壁交界处囊肿，16例蝶窦囊肿，12例真菌球性上颌窦炎，采用鼻内镜和电子鼻咽喉镜相结合进行手术治疗。结果所有患者均一次手术成功，术中出血较少，平均约（20±3）mL。术后随访6个月，复查见鼻窦自然开口均无闭锁或粘连，所有患者均未见复发。结论鼻内镜结合电子鼻咽喉镜在鼻窦手术中，尤其是对上颌窦前壁、前壁与底壁交界处，蝶窦底壁、侧壁病灶的清除，能在尽可能减少窦口开窗范围的情况下，充分发挥各内镜的优点，以达到微创目的。

简介：AIM:Tostudythedistributionofocularhigher-orderaberrations(HOAs)andmesopicpupilsizeinindividualsscreenedforrefractivesurgery.·METHODS:OcularHOAsandmesopicpupilsizewerestudiedin2458eyesof1240patientswithmyopia,myopicastigmatismandcompoundmyopicastigmatismand215eyesof110patientswithhyperopia,hyperopicastigmatismandcompoundhyperopicastigmatismusingtheZywaveaberrometer(Busch&Lomb).Allpatientshadcorrectablerefractiveerrorswithoutahistoryofrefractivesurgeryorunderlyingdiseases.Root-mean-squarevaluesofHOAs,totalsphericalaberration,totalcomaandmesopicpupilsizewereanalyzed.OcularHOAsweremeasuredacrossa≥6.0mmpupil,andpupilsizemeasurementswereperformedunderthemesopiccondition.·RESULTS:ThemeanvaluesofHOAs,totalsphericalaberrationandtotalcomainthemyopicgroupwere0.369μm,±0.233,0.133±0.112μmand0.330±0.188μm,respectively.InthehyperopicgroupthemeanvaluesofHOAs,totalsphericalaberrationandtotalcomawere0.418μm±0.214,0.202±0.209μmand0.343±0.201μm,respectively.HyperopesshowedgreatertotalHOAs(P<0.01)andtotalsphericalaberration(P<0.01)comparedtomyopes.Inage-matchedanalysis,onlytheamountoftotalsphericalaberrationwashigherinthehyperopicgroup(P=0.05).Mesopicpupilsizeinthemyopicgroupwaslarger(P≤0.05).·CONCLUSION:TheresultssuggestedthatsignificantlevelsofHOAswerefoundinbothgroupswhichareimportantforplanningrefractivesurgeriesonIranians.Thereweresignificantlyhigherlevelsoftotalsphericalaberrationinhyperopescomparedtomyopes.Mesopicpupilsizewaslargerinmyopicgroup.

简介：AIM:Toevaluatetheeffectofbrinzolamide-timololfixedcombinationonintraocularpressure(IOP)aftercataractsurgery.METHODS:Thestudyincluded92eyesof87patientswhounderwentcataractsurgeryandintraocularlensimplantation.Patientsscheduledforphacoemulsificationwereassignedto1of2groups.Thetreatmentgroupreceived1dropofbrinzolamide-timololfixedcombinationimmediatelyaftersurgery,andthecontrolgroupreceivednotreatment.TheIOPwasmeasuredpreoperativelyandat2hand24hpostoperatively.RESULTS:ThemeanIOPchangewaslowerinthetreatmentgroupthaninthecontrolgroupat2hpostoperatively.ThedifferencebetweenthemeanIOPvaluesofthetwogroupsat2hpostoperativelywasfoundtobestatisticallysignificant.Twenty-fourhoursafterthesurgery,themeanIOPchangewasstillhigherinthecontrolgroupwhencomparedtothetreatmentgroup.CONCLUSION:ThefixedcombinationbrinzolamidetimololcaneffectivelyreduceIOPaftercataractsurgery.

简介：AIM:Toinvestigatetheantifibroticeffectofthefreeze-driedbilayeredfibrin-bindingamnioticmembraneasadrugdeliverysystemonglaucomasurgeryinrabbitmodel.Theaimofthisstudywastoprepareanovellocaldeliverysystemforthesustainedandcontrollablereleaseof5-Fu.METHODS:Twenty-fourJapanesewhiterabbitswererandomizedintothreegroups:theexperimentalgroup(oculartrabeculectomyincombinationwith5-Fuloadedfreeze-driedbilayeredfibrin-bindingamnioticmembranetransplantation),thecontrolgroup(oculartrabeculectomyincombinationwith5-Fu)andtheblankgroup(singletrabeculectomy).HEstaining,massionstainingandimmunohistochemistryforα-SMAwereperformedondays7,14,21and30followingsurgery.Theconcentrationof5-Fuinrabbitaqueoushumorwasexaminedbyhighperformanceliquidchromatography(HPLC)3daysafterthesurgery.RESULTS:Statisticaldifferenceswerenotedinintraocularpressureamonggroupsonday7,14,21and30followingsurgery.Histologyfurtherdemonstratedthattrabeculectomyincombinationwithfreeze-driedbilayeredfibrin-bindingamnioticmembraneyieldedwellwoundhealingandnoscarformationandwasbeneficialforlongtermeffect.CONCLUSION:HPLCshowedagoodslow-releaseeffectwithfreeze-driedbilayeredfibrin-bindingamnioticmembrane.

简介：AIM:Toinvestigatethespecialtyoftranscranialsurgerythroughpterionalapproachforremovalofcranio-orbitaltumors,introducetheophthalmologicalexperiencesofenteringtheorbittoreducetheincidencerateofassociatedcomplicationsofthisoperation.·METHODS:Weperformedaretrospectiveanalysisofaseriesof37casesinvolvingpatientswhounderwenttranscranialsurgerythroughpterionalapproachfortreatmentofcranio-orbitaltumorsinourdepartmentinthepast8years.Pterionapproachcraniotomywasperformedtoallpatients.Afterremovingtumorsintheskullbytheneurosurgeon,ophthalmologistremovedtumorsinorbit.Wetookmeasuresbelowtodecreasecomplications,includinggroundingopticcanalthroughanabrasivedrillingwhennecessary,hangingvariousextraocularmusclestobeexposedforprotection,refrigeratingbyrefrigerationheadstoremovetumors,atlastsewinguporbitseptumaftersurgery.·RESULTS:Tumorswereremovedcompletelyin32cases,andincompletein5casesduetoextensiveinvasionintothecavernoussinusorsphenoidsinus.Ofallthecases,benigntumorsweredemonstratedin28cases(75.6%,28/37)andmalignantin9(24.3%,9/37).Themostcommonlesiontypewasmeningiomain11cases(29.7%,11/37).Extraocularmuscles(EOM)impairment,occurringin21cases(56.7%,21/37),wasthemostfrequentpostoperativecomplication.Themostseriousconsequencewasvisionlossoccurredin4cases(10.8%,4/37).Othercomplications,suchas11casesoftransientblepharoptosis29.7%(11/37),5casesofmydriasisin13.5%(5/7);2casesofcerebrospinalrhinorrheain5.4%(2/37).·CONCLUSION:Cranio-orbitaltumorscanberemovedcompletelyusingtranscranialapproach,andthepterionalapproachoffersexcellentexposure.Cooperationofinterdisciplinaryteamofneurosurgeonsandophthalmologistsconducestofulluseofrespectiveprofessionaladvantages.Theexperienceofophthalmicoperationtechnologycandecreaseoccurrenceofocularcomplicationsaftersurger
